How do you fill in the activity plan for Japan's Digital Nomad visa?
Last checked: 2026-11-06
The activity plan is a short form that says when and where you will stay in Japan, and what remote work you will do there. The Immigration Services Agency publishes a reference format in Word and PDF. You can skip it at the visa stage if you present a Certificate of Eligibility. Your office may add its own rules, so read its checklist first.
What the form asks
The form is called "Description of intended activities". It is in English and Japanese.
| Field | What to write |
|---|---|
| Date | The date you fill it in, as dd/mm/yyyy |
| Name of applicant | Surname, middle name, given name |
| Date of birth | dd/mm/yyyy |
| Phone | A number reachable in Japan |
| Your address | |
| Spouse or parent with Digital Nomad visa | Name and date of birth. This is for a family member's form. |
| Period | From and to, as dd/mm. There are several rows, one for each stretch of your stay. |
| City and prefecture | Where you plan to stay in that period |
| Description of activities | What you will do during your stay |
The form gives one example for the last field. It names the type of contract, the company, the country, the weekly hours, and tasks such as meetings and documents.
What offices add
| Office | Its rule |
|---|---|
| Houston | Fill out a detailed plan, not on a monthly basis. |
| Los Angeles | You must use this form. |
| Chicago | Describe the work, and include a phone number reachable in Japan and your timeframe. |
| San Francisco | The form is part of its checklist, if you have no Certificate of Eligibility. |
| Embassy in Finland | Print it on A4 paper. |
The Immigration Services Agency says that on its route you need not submit it separately if the application form already gives the details.
A worked example
This person and company are invented, and the plan is our own example. It is not an official sample, and it does not mean an office will accept it. It only shows how the fields fit together.
| Period | City and prefecture | Description |
|---|---|---|
| 01/04 to 30/06 | Fukuoka City, Fukuoka | Based on my employment contract with Example Software Ltd in Canada, I will work remotely about 40 hours a week on software development, including online meetings with colleagues in Canada. |
| 01/07 to 30/09 | Sapporo City, Hokkaido | Same work under the same contract. |
Houston wants more detail than this, so split the rows further if your office asks.
Five points to check
- Dates: keep the total within six months. Use the stay calculator.
- Insurance: the policy dates must cover the same period. See the insurance page.
- Work: describe work for a foreign employer, or services and sales to people abroad. Do not list a job or contract with anyone in Japan. See the requirements.
- Contracts: the company and contract you name should match your proof of work and income. See the documents list.
- Phone: ask your office what it accepts if you have no number in Japan yet. See the notes on SIM cards without a residence card.
What we could not confirm
- Level of detail: only Houston states one. Ask your office.
- Phone number: we found no rule on a home-country number that roams in Japan.
- Family members: the form has a field for a spouse or parent. We found no example of what a family member writes as activities. The status covers daily life as a supported spouse or child.
- Changes after you arrive: the Q&A says moving between cities needs no special procedure. We found nothing on updating the plan.
